Multi-Column select dialog

Has anyone written or seen a plug-in that has this functionality? I want a pop-up dialog that has a multi-column list that would let me select a row - selecting a row would fill in the row name into a form field. Something like this:

Just create a window with a ListView with a click or dblclick listener.....
