Login form with submit button

hi,
still early days on sencha touch. i want to create a Login form with username and password field and ofcourse a submit button. can anyone help me with a detailed code example. i have already search it on Forum but can't find any detailed example. there are some login form threads but still i m confused.

any early response will be by pleasure. thanks in advance.
