Just finished writing an ExtJs 4 based plugin for TinyMCE 3.x which allows you to:
1) Select Image files from the server
2) Select Media files from the server
3) Upload files to the server

The plugin is open-source and free and is available here:

you should integrate the swf upload so that you can upload more images at a time.

Hi dawesi, thanks for the feedback. SWF upload is not really required for uploading more images at a time. This can be done in pure ExtJs also (see my Take Control module's demonstration (http://drupal.sandbox.rahulsingla.com/admin/build/take_control) for example, click Upload on the toolbar).

What if you have 20 images? It's not "best" solution.
But very good work

Hi morfeusz, thanks for the feedback. As I said, I assembled this plugin very quickly for one particular project and published it open-source in its current form. To make it more flexible, we can easily integrate this:

http://www.rahulsingla.com/blog/2011/04/html-using-buttons-for-selecting-files-to-be-uploaded

with the Upload Panel we have here (click Upload on the toolbar):
http://drupal.sandbox.rahulsingla.com/admin/build/take_control

to create seamless Flash upload kind of an effect.

Hi... this looks really nice. I would love to see it be able to show thumbnail images when choosing which image file to insert... instead of just the image file names. And would also love to see the ability to delete an image on the server. Those two would make this AWESOME!!! Hope you visit it again soon and consider the suggestions.

Hi Wayne, thanks for the feedback. I might NOT visit it again. I would suggest you have a look at these links which should help you integrate and assemble most of what you need:
