Help starting a new HtmlEditor plugin

I'm having trouble creating a new plugin for the HtmlEditor component. I need to build a custom upload/browse image and although they have it for 3.x, it doesn't work on 4.x.

Could someone point me to adding a plugin to the HtmlEditor Toolbar?
