Help with Form in JS

Hi,
Im having trouble styling a form, i want to make something like this:

But everything i've tried didn't work so far, here is my last attempt:

I want to know, what do i have to do, to get something similar to the example, also i want the form to adjust to the window size, but i don't know how to make
this happen.

Two things I see.. the border around each column and the field label align to the left.

The border of each column is caused because you are probably wrapping the fields in a panel. Try it with a container.

The field label alignment is simple, on your fields (more in the defaults object of the container) set the labelAlign to 'right'
