How to place a logo at the top of a form ?

Hello everyone,
I'm working with ExtJS 4.1, my task is to create a form with customer's logo at the top.
The frame on which I'm writing my own code is this example, from Sencha's website:

What I'd like to do is placing customer's logo into the form of this example, above email address field, as form's header.
Can anyone suggest me a good way to do that ?
