One thing to keep in mind about custom form solutions like the one linked above is that they tend to disrespect common accessibility standards. For example, the dropdown controls do not properly respond to keyboard commands and you cannot select items using keyboard navigation. Also, when tabbing through the form, the radio buttons give no visual clue that they have focus, which is confusing. Yes, they look nice, but be aware of the trade-offs too.
