What is the difference between the "commercial evaluation" download and the "GPLv3 for open source" download?

The difference is that under a GPL v3 license, the code is freely distributed, and you may build whatever you like on it, provided you are willing to make your source available under the terms of the GPLv3 (or under other licenses through exceptions we have provided, as indicated on licensing page http://www.sencha.com/products/extgwt/license/). With the commercial license, there are no licensing requirements for your code. We request your email address when you try out the commercial version so we can see how that evaluation is going. If you pursue the commercial license, you will have access to subscriber support, as well as SVN access to the library.
The downloads should be the same, the difference is mostly to indicate that there is a difference in how the two licenses apply.
