You can use Sencha Cmd to generate the index for you, or you can simply use:

./resources/css/ext-all.css
ext-all.js / ext-all-debug.js ( minified / un-minified )

You would need to be more specific. The links that you posted were for your local server. ( not accessible online )

Here is a basic html

<html>
<head>
<meta http-equiv="Content-Type" content="text/html; charset=utf-8" />
<title>MyApp</title>

<link rel="stylesheet" type="text/css" href="extjs4/resources/css/ext-all.css"/>
<link rel="stylesheet" type="text/css" href="extjs4/examples/shared/example.css" />

<script type="text/javascript" src="extjs4/ext-all-debug.js"></script>
<!-- page specific -->
<script type="text/javascript" src="myapp.js"></script>
</head>
<body>
<h1>MyApp</h1>
</body>
</html>

You can run from a local web server, apache, etc.
