I checked Grid API and did not find a way how to create a header containing more than one row.
Is it actually possible?
If it is not I am very surprised as this is so fundamental feature which is used quite often.
In addition multi-row header could be useful to display all table totals at the top of the table.

There is support for summary grouping and totaling based on that.

Yes, but it does not cover many real cases.
If we do not use Grouping then it makes much more sense to have column totals in the non-scrollable area (like header or footer) so they are always visible and users do not need to scroll to see them. This is also true for paging table.
And header column grouping is very often used to provide user more information how data relates to each other. It could be like in gwt-incubator table sample http://google-web-toolkit-incubator.googlecode.com/svn/trunk/demo/PagingScrollTable/index.html
I believe it is very difficult currently to create such header or footer in GXT :(
