    Default "" added to index.html

    You can see below that Sencha Cmd v5.0.1.219 did 3 different things that are wrong:
    1- Added  to index.html, maybe this is bug related to encoding, and may represent some kind of UTF encoding.
    2- Duplicated meta http-equiv
    3- Default meta http-equiv is not best practice. Since Ext JS 5 latest version doesn't support IE above 11, mine is more appropriate. Sencha Cmd should ask the Ext JS SDK, what is the latest version supported and set this up.

    Still happening with Sencha Cmd v5.0.1.231, and noticed this after the following 3 actions:

    Code:
    sencha app upgrade -noframework
    sencha app build
    sencha app watch

    Still happening with Sencha Cmd v5.1.1.45 and Ext JS 5.1.1.215.
